Two or more concurrent advanced therapies, at least one being a biologic.
Targeted oral therapies (JAK, TYK2, PDE4) count toward the total.
No direct identifiers
No names, dates, or record numbers are requested at any stage

Complete the form to submit a case with no direct patient identifiers. All submissions undergo structured quality review before inclusion in the dataset.
Do not include direct patient identifiers: names or initials, dates of birth, exact treatment or visit dates, medical record or health-card numbers, addresses, contact details, or images. Free-text fields must not contain identifying details either.
What to Submit
- Patient age band and biological sex
- At least two concurrent advanced therapies, at least one of which is a biologic. Targeted oral therapies such as JAK, TYK2 and PDE4 inhibitors count toward the total
- Primary and comorbid diagnoses (ICD-10 codes preferred; specialty-specific and custom terms accepted)
- Duration of concurrent therapy
- Validated disease activity measures where available, as absolute scores at baseline and follow-up
- Clinical outcomes and adverse events observed
